Creating Web Forms
Luminate CMS web forms allow you to gather information from
constituents such as contact information or questionnaire and survey results. They also enable
constituents to create and update their profile in Constituent360, and to display user-generated content on your site. There are four types of forms: Save
Data, Create Content, Create Constituent Profile,
and
Update
Constituent Profile.

Using the Form Console - Explains how to use the Form Console which
is used with all form types.
- The Save Data Form
- Allows you to gather data from constituents that can be downloaded as a CSV
file. This is most commonly used for surveys and questionnaires.
- The Create Content Form
- Allows constituents to submit stories, upload images, and so on to display on
your site (user-generated content). When a constituent submits a Create Content
form, a new content item is created.
- The Create Constituent Profile Form -
Allows constituents to create a new profile in Constituent360
through a Luminate CMS page.
- The Update Constituent Profile Form - Allows
constituents to update their profile in Constituent360 through a
Luminate CMS page.
Polls and Author Entry Forms
Polls and Author Entry Forms are also forms, but they are not created using a form type.
- The Poll Component - Polls are a type
of form, but they are added with the Poll component rather
than a form type.
- The Author Entry Form
- Allows system administrators to enter the properties of a content item. The
Form Console for Author Entry Forms has slightly different options than for
constituent-facing forms.
